Pediatric OT Referral
We're here to make referring a child to occupational therapy simple.
Mind Body Foundations OT provides individualized, play-based occupational therapy for children from infancy through the teen years throughout the Greater Orlando/Oviedo/ Lake Mary/ Central Florida area.
We welcome referrals for concerns involving sensory processing, fine-motor development, school participation, self-care, coordination, regulation, attention, and other functional skills.
Provider referrals are reviewed promptly. After receiving the referral, we will contact the family regarding next steps.

What happens after you submit a referral?
1. Submit the referral
Complete the short provider referral form below.
2. We review the referral
Bonnie will review the information and contact the family to discuss next steps.
3. We coordinate with the family
We'll gather any additional information needed and discuss scheduling, insurance, Step Up For Students or self-pay options.
4. Evaluation & communication
Following the evaluation, appropriate information can be shared with the referring provider with the necessary authorization/consent.
